The 14th Annual Family Medicine Midwest Conference will be held on September 12-13, 2025, at the NIU Naperville Conference Center located at 1120 E. Diehl Rd., Naperville, IL.

The specific date and time of your presentation have been communicated to the person listed as the Primary Speaker in your abstract submission form. Upon arriving, follow the signage to the FMM Registration Desk, where you will check in and receive your badge.

PLEASE NOTE: The room location is subject to change; therefore, please check the FMM App once you are on-site.


All SESSION Primary and Co-Speakers are required to complete the Speaker Agreement (SESSIONS) (DUE JULY 7, 2025), and it serves as confirmation that you accept the opportunity to present your session.

All POSTER Primary and Co-Speakers are required to complete the Speaker Agreement (POSTERS) (DUE ONE WEEK AFTER INVITES ARE SENT), and it serves as confirmation that you accept the opportunity to present your poster.


First, read the IAFP's Standards for Commercial Support and Disclosure Policy, then complete the online CME Disclosure Form by JULY 7, 2025 (for SESSION Speakers).

PLEASE NOTE: All Primary and Co-Speakers for ALL SESSIONS AND POSTERS are REQUIRED to complete this form to be listed in the onsite program guide.


Within the first two slides of your presentation, all presenters/speakers, whether a Primary or Co-Speakers, are required to include their disclosure information. For a template, please click here. You may update this to reflect your slide template.

Attendee distribution: All presentations will be distributed to the 2025 FMM Attendees through the FMM App. Should your presentation contain proprietary information that you do not want distributed, you may send an edited version of the presentation to be posted. You MUST still send the full version of your presentation to the CME Committee for review and to be pre-loaded on the conference computers.

PLEASE NOTE: All presentations must be sent to Randie Salerno in their original (i.e., PowerPoint) format no later than AUGUST 18, 2025. Presentations will be converted to PDF files before loading them onto the FMM App.


Laptops and projectors will be provided for presenters and will be pre-loaded with all presentations. Each room will also have a podium, and larger rooms will have a podium microphone. Flip charts will be available for pickup at the Help Desk. No additional A/V will be provided. Should your presentation require internet or speakers for sound, please indicate these needs upon filling out the Speaker Agreement (please see above).


These boards are mounted on stands and feature a double-sided design to accommodate two posters (one on each side).

For planning your poster: (1) Individual poster size: Presenters are usually advised to create posters no larger than 3.5 feet high × 7.5 feet wide (42" × 90") to allow for a margin around the edges. (2) Orientation: Posters should be in landscape orientation.

Push pins will be provided. Poster Presenters are welcome to hang their posters 30 minutes before their presentation begins and must remove them at the close of their presentation. Poster Presenters are required to stay at their posters for the duration of their assigned time. Poster Presenters are required to complete the CME Disclosure Form and Speaker Agreement (POSTERS) (please see above).

PLEASE NOTE: As the posters are eligible for CME credit, a digital copy of your poster must be sent to Randie Salerno by AUGUST 18, 2025, for review by the CME Committee. Poster Presenters are responsible for the printing and all associated costs related to their poster.


One of the most important aspects of FMM is providing a wonderful networking opportunity for those in the family medicine community. As such, Speakers are required to register, pay (unless receiving student support funding), and attend the Conference. An early bird discount is available until August 1, 2025, with the discount code JUMPSTART.
