Founded in 2012, Family Medicine Midwest is a collaborative effort across multiple states, committed to building a strong family medicine workforce to deliver high-quality, comprehensive care to our region's residents. We focus on attracting, retaining, and developing exceptional family physicians for the future. The Family Medicine Midwest Foundation, a 501(c)(3) organization, is a recognized leader in collaborative family medicine workforce development. The Illinois Academy of Family Physicians serves as the management organization for the Family Medicine Midwest Foundation.

Family Medicine Midwest is dedicated to attracting, retaining, and developing exceptional family physicians in the Midwest. We acknowledge that systems of oppression have caused harm in historically marginalized communities, including but not limited to indigenous peoples and their occupied lands, communities of color, LGBTQ+ communities, people of varying abilities and body sizes, immigrant communities, and others who have been historically excluded from medicine. These systems of oppression are ingrained in our public health, health care, and medical education systems, where family physicians live and work. At FMM, we are committed to continually educating ourselves and our colleagues about how systems of oppression affect our work as family medicine educators, including gathering information, identifying disparities in our activities, and understanding their root causes. We understand that we will make mistakes along the way, but we will challenge ourselves to grow, learn, and take action.
