Foundation
The Family Medicine Midwest Foundation, a 501(c)(3) organization, is a recognized leader in collaborative family medicine workforce development. Our mission is to shape the future of family medicine in our region. The states include Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Nebraska, North Dakota, South Dakota, and Wisconsin. Because the 2026 conference is in Indiana, students at Ohio medical schools are invited to FMM.
The Illinois Academy of Family Physicians serves as the management organization for the Family Medicine Midwest Foundation.
