*Only students currently enrolled in schools located in IA, IL, IN, KS, MI, MN, MO, NE, ND, SD, WI
Physician - $350.00 
Resident Physician - $250.00 
**Unmatched Medical School Graduate - $250.00
**IMGs must have current ECFMG certification and 2026 Match eligibility
***Non-Midwest Medical School Student - $250.00

Accreditation Statement:
The Illinois Academy of Family Physicians (IAFP) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

Credit Designation Statement:
The AAFP has reviewed 2025 Family Medicine Midwest Conference and deemed it acceptable for up to 12.50 Live AAFP Prescribed credit(s). Term of Approval is from 09/12/2025 to 09/13/2025. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
